Complete this form and submit it before beginning any research. Submit the form to the Chair of the Institutional Review Board (IRB). 1. If your project involves research of any of the following types, it is exempt from review by the IRB, with the exceptions noted. (a) Invertebrates. Needless harm to animals is avoided. For example, pinning is not used to hold live insects in place when another method of restraint could be used. Animals are anesthetized by standard methods, such as CO2 narcosis or cold. Euthanasia is by freezing or standard chemical methods (ethyl acetate, ethanol, acetone). Species to be collected are not threatened or endangered. (b) Vertebrates. Animals will experience no physical harm or psychological stress. If animals are euthanized, the methods listed below are followed. I. Fish: Euthanasia by freezing. II. Amphibians: Euthanasia by pithing, Tricaine methane sulphonate (MS 222) immersion, barbiturate overdose (ip), or freezing. III. Reptiles: Euthanasia by pithing, barbiturate overdose (ip), or freezing. 2. If your project involves any of the following, it is subject to expedited review: Blood or other biological samples are collected by standard techniques. Minor psychological stress is expected (e.g. training). If euthanasia is planned, the methods listed below are followed. (a) Mammals For euthanasia of rodents and rabbits, carbon dioxide asphyxia, barbiturate overdose (iv or ip) and anesthetic overdose. In anesthetized animals only, decapitation, cervical dislocation and exsanguination. (b) Birds For euthanasia, barbiturate overdose (iv or ip), anesthetic overdose, carbon dioxide asphyxia, and, in anesthetized animals only, decapitation, cervical dislocation and exsanguination. Submit a brief description of your project, along with your signature. and the signature of your faculty member on this form to the IRB to document your exemption from IRB review. The brief description of your project should include the type of animals, the personnel administrating the project, the information to be gathered and the means of collecting data, the location of the project, and its duration. Begin your research after receiving approval from the IRB. 3. If your type of project is not listed in #1 and #2, does it involve any of the following subjects or categories? ___ threatened or endangered species ___ moderate to extreme psychological stress ___ invertebrates: laboratory methods not listed in 1(a) above ___ vertebrates: anesthesia or surviving surgeries ___ vertebrates: trapping or other obtrusive field research ___ birds and mammals: euthanasia using methods not listed in 2(a) above ___ farm animals ___ primates ___ funding by the United States Public Health Service. If you answered no to all of the above questions, submit a brief description of your project (see guidelines in #1 2 above), along with your signature on this form to the IRB to begin the expedited review process. Begin your research after receiving approval from the IRB. For research involving human subjects or cadavers, see separate policy statements. If you answered yes to any of the above questions, submit this form to the IRB along with an additional explanation of why these animal subjects or categories are included, what benefit you expect your project to produce, and any ethical concerns that might be raised about it. Your project will undergo full review by the IRB, and you must await its approval before beginning your research. The IRB, at its discretion, retains the right to require continuing review when warranted by the nature of the research and/or inclusion of particular species.
